Ten more senior living and care organizations in Minnesota have joined the provider network of an HMO institutional special needs plan, a targeted insurance program aimed at improving care, reducing costs and providing navigation services.
The Medica Advantage Solution– PartnerCare plan from nonprofit health plan Medica and geriatric medical practice and care management organization Genevive is now available to residents of the following providers:
- Assumption Community
- Cokato Charitable Trust
- Ecumen
- Good Shepherd Community
- Lyngblomsten
- St. Francis Health Services
- St. Otto’s Care Center
- Shakopee Friendship Manor
- Sholom Home
- Sterling Park
The 10 new organizations join 11 existing providers in the plan, most of which were announced in 2019 for the inaugural 2020 plan year. At the time, the collaborators believed it was one of the largest rollouts of an I-SNP in the country, with the potential to serve 5,500 residents.
Altogether now, according to Medica, eligible residents of 140 individual senior living and care communities are able to select the Medicare Advantage I-SNP for 2021 coverage if they need or are expected to need the level of services provided in a long-term care facility for 90 days or more. With the addition of the new communities, the geographic reach of the plan also expands from seven counties to 12.
The plan is designed expressly to meet the often complex health needs of residents of assisted living and memory care communities as well as other long-term care settings.
Genevive is a stand-alone healthcare organization co-owned by Allina Health and Presbyterian Homes & Services. The organization partnered with nonprofit health plan UCare to design two HMO I-SNPs, UCare Advocate Choice and UCare Advocate Plus, which on Jan. 1 began serving residents of Minneapolis / St. Paul metropolitan area long-term care communities that are part of 22 participating organizations within a 14-county metro service area.
Among other Medicare Advantage plan special partnerships that have formed in recent years are Toledo, OH-based real estate investment trust Welltower’s collaboration with CareMore and Bloomfield, NJ-based Juniper Communities’ partnership with AllyAlign Health.
