Humana, an insurer and home care provider, reportedly is launching a pilot platform — Humana Care Support — for Medicare Advantage members with multiple chronic diseases in Pennsylvania, West Virginia and Kentucky.

The program will use enhanced data analytics to enable customized care, and will provide members with access to multidisciplinary care teams that include a nurse, pharmacist, social worker and behavioral health expert, according to the Fierce Healthcare news outlet. The pilot is built on Salesforce’s Health Cloud platform, allowing access to the member’s full medical history and any integrated clinician workflow. Care teams can track COVID-19 patients to monitor their health and customize their care. Humana noted that members now expect this level of personalized care.

Humana, which owns Humana at Home and Kindred at Home — the largest home health provider — has taken other steps to provide hospital level care at home. It recently formed a partnership with DispatchHealth, a provider of in-home medical care, to provide members living with multiple chronic conditions an opportunity to be treated safely at home and thereby avoid hospital visits.
